Configurable manual and AED shock levels with shock escalation

This allows a default shock energy to be set as well as automatic shock escalation as featured in some devices.

To configure these settings open REALITi on the Monitor iPad and select Settings>Monitor>Device (such as Lifepak 15, etc)

Electrode placement shortcuts

The Heart Rate and ECG control box has been improved for easier access to Pads on/off and ECG electrode selection.
